Hospital discharge is the process of preparing and facilitating a patient so that they can leave hospital. In principle all people being discharged should have been considered for a discharge plan with a proper [[needs assessment conducted where necessary. This should have been discussed with them to ensure that they have all the social support needed to get home and settled in and that any health care needs and social service needs are identified and the appropriate aftercare will be delivered.

Issues in integrated care[]

One of the challenges of integrated care is to cordinate the community services so that they are prepared to pick up the care of vulnerable people once they leave hospital.
